Comprehensive Assessment and Care Plan
In person meeting with individual and their family/support to complete a comprehensive assessment (at home or at facility for those considering returning home)
Assessment focuses on the person’s functional and cognitive abilities, home safety, and the goals of the both the individual and family members.
Aging Care Solutions will develop and provide a written care plan, identifying the person’s needs and potential solutions.

Assist family members in providing the support they would if time and proximity allowed
